Application for Registered Nurse by Examination

Fees: $350. This fee covers the following:

  • Application for licensure fee: $300.00
  • Fingerprinting fee: $50.00

All fees must be in US dollars and are non-refundable.

Highlights:
Application forms for the state of Arizona cannot be submitted online. You must print out the form, fill it out, and mail it in to the Arizona Board of Nursing.

Submitting the application for licensure by examination does NOT register you for the exam. Applicants must register and pay separately for the exam by going to: www.pearsonvue.com.

The state of Arizona requires documentation of citizenship, nationality, or alien status from ALL applicants. If you are not a U.S. citizen, national, or have qualifying alien status, you cannot be licensed as a nurse in the state of Arizona.

Before mailing the application in:

  • Request a Certificate of Completion, showing that you completed an approved nursing program (if an Arizona graduate). The certificate of completion should be sent by your school directly to the Board of Nursing.
  • Request official transcripts from your school (if graduated from a program outside Arizona). The transcripts should be sent by your nursing school directly to the Board of Nursing.
  • Attach evidence of your citizenship or alien status. To do this, you must make photocopies (Xeroxed copies) of acceptable identification documents. The copies must be on 8 1/2 x 11 paper and submitted with the application. Look at pages 14, 15, and 16 of the application form to see a list of acceptable documents.

NOTE: SOCIAL SECURITY CARDS ARE NOT ACCEPTABLE AS DOCUMENTATION OF CITZENSHIP OR ALIEN STATUS.

Fingerprints:
Once you have submitted the application to the Nursing Board, a fingerprint card will be mailed to you. You must use the exact card that is sent to you since it has specific agency data pre-printed on it. You may complete the fingerprint card at any fingerprinting agency. NO licenses will be issued until fingerprints have been processed (this takes 2-3 weeks).

About Temporary Permits

A temporary permit allows you to practice nursing until the results of the NCLEX are available.

The state of Arizona does NOT issue temporary permits. The exceptions are:

  • If you have already passed NCLEX, but your fingerprints are rejected.
  • If more than 2 years have passed since you graduated and must take a refresher course to become licensed.
