Application forms for the state of Florida should be submitted online at the link given above. Paper applications are also accepted.

If you use a paper application you may fill it out on the computer, but you must print it, sign it (no typed signatures are accepted) and mail it in.

To submit the application you will need: A valid Social Security number, and a physical mailing address.

Submitting the application for licensure by examination does NOT register you for the exam. Applicants must register and pay separately for the exam by going to: www.pearsonvue.com.

As of January of 2013 the state of Florida requires electronic fingerprinting. Non- electronic fingerprint cards are no longer accepted.

The state of Florida requires that you complete a Florida-approved 2 hour course in the Prevention of Medical Errors. This course may have been included in your nursing curriculum. If you have not completed this course, you will not be authorized to test. You may search for courses to satisfy this requirement through CE Broker at www.cebroker.com/

As part of the application process, you must:

  • Make sure you have completed an approved 2 hour course in the Prevention of Medical Errors.
  • Request verification that you completed a nursing program. Verification must be sent by your nursing school directly to the Board of Nursing

  • Register with Pearson Vue to take the NCLEX exam. It is absolutely necessary that the name you use to register for the examination and the name used on the licensure application are exactly the same.

  • Submit the online or paper application for Licensure by Examination along with the fee and any supporting documents.

  • Schedule an appointment for electronic fingerprinting.
  • About the Electronic Fingerprinting Process:

  • The applicant is responsible for finding a Livescan service provider (vendor) and for any fees or costs associated with the fingerprinting process.
  • Important: You must provide the Livescan vendor with an ORI number. This is also known as an Originating Agency Identification Number. The ORI number lets the vendor know what agency is requesting the fingerprint report.
  • When you complete the online application process, you will be directed to print a form containing the ORI number. It is essential that you take the printed form, with the ORI number, to your fingerprinting appointment.
  • If you are using a paper application, the form with the ORI number is found on page 27. You must take this page to your fingerprinting appointment.
  • You will be required to bring two forms of identification, including one with your picture on it, such as a driver’s license. You may also need to bring a Social Security card.
  • If the Livescan vendor does not take your photograph as part of the fingerprinting process, you may be required to repeat the process with another vendor.
  • The fingerprints should be electronically submitted to the Florida Department of Law Enforcement. They will be used to determine if you have a criminal history. A report will then be sent to the Nursing Board.

Licensing Process

Once the application, supporting documents, and criminal background report have been received, the Board will determine if you are eligible to test.

When you have been notified that you are eligible to test, you may schedule your examination.

You must take the NCLEX within 90 days after receiving the "Eligibility to Test" notification.

Once passing test results are reported to the Board of Nursing, licensure is issued within 7-10 days.

About Temporary Permits:

New RN and LPN graduates may practice as Graduate Nurses (G.N.) or Graduate Practical Nurse (G.P.N.) before taking the NCLEX only if they have received approval from the Board of Nursing. This approval is given after the application for licensure and supporting documents have been reviewed, and you have been found eligible to test.

Accommodations:
If you require special testing accommodations due to disability or religious conflict, you must fill out the Application for Special Testing Accommodations found here: Special Accommodations Request Form. This is in addition to the application for licensure by examination submitted to the Board of Nursing. You should be aware that your application for special accommodations must be submitted 60 days prior to the examination date.
